Urinothorax: A rare cause of pediatric pleural effusion

ABSTRACT

A 16-year-old female with a complex medical history, including myelomeningocele and nephrolithiasis, presented with severe chest pain and dyspnea following nephrolithotomy and nephrostomy tube insertion. Chest X-ray and CT scan revealed a unilateral pleural effusion, while laboratory studies revealed elevated levels of creatinine in pleural fluid, thereby establishing the diagnosis of a urinothorax. This is an unusual cause of pleural effusion, caused by either an obstructive uropathy or direct injury to the pleural cavity. Treatment is largely symptomatic, with consideration of thoracentesis for large effusions. Urinothorax should be considered as a cause of pleural effusion in patients.

RÉSUMÉ

Une jeune fille de 16 ans aux antécédents médicaux complexes, dont une myéloméningocèle et une néphrolithiase, présentait une dyspnée et une violente douleur à la poitrine suite à l'insertion d'un tube en vue d'une néphrolithotomie et d'une néphrostomie. Une radiographie et une tomodensitométrie du thorax ont révélé une effusion pleurale unilatérale, tandis que les études en laboratoire ont révélé des niveaux élevés de créatinine dans le fluide pleural, ce qui a permis d’établir un diagnostic d'urinothorax. Il s'agit d'une cause inhabituelle d'effusion pleurale, occasionnée soit par une uropathie obstructive, soit par une blessure directe à la cavité pleurale. Le traitement est en grande partie symptomatique et la thoracentèse est envisagée pour les grandes effusions. Ce diagnostic devrait être envisagé en tant que cause de l'effusion pleurale chez les patients qui souffrent de troubles rénaux ou suite à des procédures invasives.
