Complications of pulmonary artery aneurysms in pulmonary arterial hypertension

ABSTRACT

Pulmonary artery aneurysm (PAA) is a rare complication of pulmonary arterial hypertension (PAH), often occurring in patients with long-standing disease. Complications of PAA include pulmonary artery rupture, thrombosis, pulmonic valve regurgitation and coronary artery compression, which can result in myocardial ischemia. Diagnosis of coronary artery compression due to PAA is based on suggestive symptoms, imaging and coronary angiography. We present a case of a 70-year-old Caucasian female with longstanding vaso-reactive PAH complicated by a large PAA compressing the left anterior descending (LAD) coronary artery. The compressed LAD was not amenable to coronary stenting, however due to extensive coronary collateralization she was managed medically with dual antiplatelet therapy and cardiovascular risk reduction, with improvement in symptoms and stability for over 2 years.

RÉSUMÉ

L'anévrisme de l'artère pulmonaire (AAP) est une complication rare de l'hypertension artérielle pulmonaire (HTAP) qui survient souvent chez les patients qui souffrent de cette maladie depuis longtemps. Les complications possibles de l'AAP comprennent la rupture de l'artère pulmonaire, la thrombose, la régurgitation pulmonaire à travers la valvule et la compression de l'artère coronarienne, pouvant entrainer une ischémie myocardique. La compression de l'artère coronarienne en raison d'un AAP peut être diagnostiquée par des symptômes suggestifs, par imagerie et par angiographie coronarienne. Nous présentons le cas d'une femme caucasienne de 70 ans souffrant d'une HTAP vasoréactive de longue date, compliquée par un gros AAP qui comprimait l'artère coronarienne descendante (ACD) gauche. Toutefois, l'ACD comprimée ne se prêtait pas à la pose d'endoprothèses coronariennes en raison d'une importante collatéralisation. Elle a été prise en charge médicalement à l'aide d'un traitement antiplaquettaire et de réduction du risque cardiovasculaire. Ses symptômes se sont améliorés et stabilisés pendant plus de deux ans.
