Hemoptysis after exposure to a household insecticide spray: A case report and review of the literature

Abstract

We describe a case of chemical pneumonitis with alveolar hemorrhage after acute exposure to a common household pyrethrin spray in a patient on anticoagulation with underlying chronic obstructive pulmonary disease (COPD). We review the relevant literature pertaining to adverse respiratory effects of pyrethrin based insecticide spray exposures. This case report highlights the importance of including potential inhaled household respiratory irritants in the differential for patients that present to the emergency department (ED) in respiratory distress. While aerosolized pyrethroid and pyrethrin containing domestic products generally do not cause significant respiratory symptoms when accidentally inhaled, the risk of serious adverse events may be increased in those patients with preexisting respiratory disease or who have used the product within a confined space. Timely recognition of adverse respiratory events related to pyrethroid or pyrethrin inhalation, airway protection and supportive care are the pillars of treatment for these patients.

RÉSUMÉ

Nous décrivons un cas de pneumonie avec hémorragie alvéolaire après une exposition aiguë à un aérosol à base de pyréthrine d’usage domestique commun chez un patient sous traitement anticoagulant avec MPOC sous-jacente. Nous avons examiné la littérature pertinente portant sur les effets respiratoires indésirables des expositions aux insecticides en aérosol à base de pyréthrine. Ce rapport de cas met en évidence l’importance d’inclure dans le différentiel les irritants respiratoires d’usage domestique potentiellement inhalés par les patients qui se présentent aux urgences en détresse respiratoire. Bien que les produits domestiques contenant de la pyréthrinoïde et de la pyréthrine en aérosol n’entraînent généralement pas de symptômes respiratoires importants lorsqu’ils sont inhalés accidentellement, le risque d’évènements indésirables sérieux peut être plus élevé chez les patients qui souffrent d’une maladie respiratoire préexistante ou qui ont utilisé le produit dans un espace confiné. La reconnaissance en temps opportun des évènements respiratoires indésirables liés à l’inhalation de pyréthrinoïde ou de pyréthrine, la protection des voies respiratoires et des soins de soutien sont les piliers du traitement pour ces patients.
