Successful hematopoietic stem cell transplantation for acute myelomonocytic leukemia in a patient with active pulmonary tuberculosis

Abstract

A 45-year-old woman presented with acute myelomonocytic leukemia (AMML) with a 54% blast count. The patient developed frank hemoptysis during cycle 1 of consolidation therapy with cytarabine and midostaurin following induction chemotherapy with cytarabine and idarubicin. Sputum collected at that time was mycobacterial smear negative but culture identified drug-sensitive pulmonary tuberculosis (TB). Since she had AMML with a high risk for relapse, allogeneic hematopoietic stem cell transplant (HSCT) was planned. In consultation with TB services, the transplant occurred after 45 days of TB treatment. We report a successful HSCT in a patient while undergoing treatment for active TB.

RÉSUMÉ

Une femme de 45 ans a présenté une leucémie aiguë myélomonocytaire (AMML) avec 54 % de blastes. La patiente a développé une hémoptysie franche au cours du cycle 1 du traitement de consolidation par cytarabine et midostaurine après une chimiothérapie d’induction par cytarabine et idarubicine. Les crachats collectés à ce moment-là étaient négatifs pour les frottis mycobactériens mais la culture a identifié une tuberculose pulmonaire (TB) sensible aux médicaments. Comme elle avait une AMML avec un risque élevé de rechute, une allogreffe de cellules souches hématopoïétiques (GCSH) était prévue. En consultation avec les services antituberculeux, la greffe a eu lieu après 45 jours de traitement antituberculeux. Nous rapportons une GCSH réussie chez un patient sous traitement pour une TB active.
