Redefining asthma management: Remodeling, rehabilitation and remission

Abstract

Addressing the multifaceted aspects of asthma care calls for a thorough examination of new evidence regarding additional effects of treatments, as well as pioneering a paradigm shift in treatment outcomes.

The concept of “people remodeling” unveils the complex interplay between changes in body weight and the use of oral corticosteroids, particularly highlighting how obesity, often a consequence of medication side effects, significantly influences asthma management. Simultaneously, integrating exercise into asthma treatment has undergone a transformative evolution, shifting from careful consideration to a pivotal cornerstone. Within the landscape of severe asthma treatment, the introduction of biological therapies has triggered a profound revolution by presenting the possibility of achieving clinical/complete remission, representing a departure from the conventional control goal.

This review will discuss recent articles focusing on weight loss associated with biologics in patients with severe asthma, the effects of exercise on lung function in asthmatics and a consensus definition of clinical remission in severe asthma.

La prise en charge des aspects multiples de l'asthme nécessite un examen approfondi des nouvelles données concernant les effets supplémentaires des traitements, ainsi qu'un changement de paradigme dans les résultats des traitements.

Le concept de « remodelage humain » dévoile l'interaction complexe entre les changements de poids corporel et l'utilisation de corticostéroïdes oraux, soulignant en particulier comment l'obésité, souvent une conséquence des effets secondaires des médicaments, influence de manière significative la prise en charge de l'asthme. Simultanément, l'intégration de l'exercice physique dans le traitement de l'asthme a connu une évolution transformatrice, passant d'une considération prudente à une pierre angulaire essentielle. Dans le paysage du traitement de l'asthme sévère, l'introduction des thérapies biologiques a déclenché une profonde révolution en offrant la possibilité d'obtenir une rémission complète/clinique, ce qui représente une rupture par rapport à l'objectif de contrôle conventionnel.

Cette revue abordera des articles récents portant sur la perte de poids associée aux médicaments biologiques chez les patients souffrant d'asthme sévère, les effets de l'exercice sur la fonction pulmonaire chez les asthmatiques et une définition consensuelle de la rémission clinique dans l'asthme sévère.
