The authors have been made aware of an error on the fifth page of the manuscript, in the last sentence of the Results section. The sentence should read:
Using the RW event rates for warfarin exposure observed in the Medco population as reference rates, we estimate a NNT of 91 for stroke and a NNT of 30 for major bleeding prevented per year.
This error did not affect any of the study results or conclusions.
