Tubular Reabsorption of Phosphate in Nephrolithiasis and Hyperparathyroidism

Abstract

The serum calcium, serum phosphate, alkaline phosphatase, serum creatinine, renal tubular reabsorption of phosphate, and phosphate excretion index were determined in 496 patients showing features suggestive of nephrolithiasis or hyperparathyroidism. Among these patients 365 had renal or ureteral calculi, 99 had other diseases of the urinary tract, 14 showed roentgen signs of cystic bone demineralisation, and 18 had miscellaneous diagnoses. Hyperparathyroidism was verified in 9 of 11 cases after surgical exploration of the parathyroid glands. Determination of the serum calcium appeared to be the most reliable single screening and diagnostic test for hyperparathyroidism. Estimation of the tubular reabsorption of phosphate seemed to be of some, though secondary, value—whereas analyses of serum phosphate, alkaline phosphatase activity and calculation of the phosphate excretion index were of no significance to differential diagnosis.
