The authors have requested the following should be noted as this was incorrect in their original submission:

‘Time to onset of symptoms” in patients who survived should be 9.7 ± 8.3 hours instead of 13.1 ± 14.0 hours. They did not find an error in the results obtained from all patients and patients who died. Row related to “time to onset of symptoms” in Table 1 must be as follows:
