A learning assessment procedure as a test supplement for monitoring progress with two post-coma persons with a diagnosis of vegetative state

Abstract

Objective: Evaluating a learning assessment procedure for monitoring progress with two post-coma adults with a diagnosis of vegetative state.

Method: ABABCBCB and ABABCB designs were used for the two participants, with A representing baseline, B intervention and C control conditions. Participants’ activation of an optic microswitch by eyelid closure produced stimulation during B phases.

Results: One participant increased responding during B phases and decreased it during the C condition, suggesting a non-reflective minimal level of consciousness. She showed P300 and mismatch negativity responses and scored at the vegetative level on the Coma Recovery Scale-Revised (CRS-R). The other participant increased responding during the initial B phases without decline during the first (viable) part of the C condition, suggesting a pre-conscious level. He showed indistinct P300 and mismatch negativity responses and vegetative-level scores on the CRS-R.

Conclusion: Learning data seemed reconcilable with neurophysiological measures and more positive than CRS-R scores.

Objetivo: Evaluar un proceso de aprendizaje para monitorizar progreso con dos adultos post-coma con diagnóstico de estado vegetativo.

Método: Se utilizaron diseños ABABCBCB y ABABCB para los dos participantes, donde A representaba la basal, B intervención, y C condiciones Control. La activación de un microinterruptor óptico por medio de la oclusión palpebral produjo estimulación durante las fases tipo B.

Resultados: Un participante incrementó sus respuestas durante la fase B y las disminuyó durante las fases C, sugiriendo un nivel mínimo de conciencia no reflectiva. Ella mostró P300 y respuestas de discordancia negativas y se ubicó en el nivel vegetativo de la Escala Revisada de Recuperación del Coma (Coma Recovery Scale-Revised, CRS-R). El otro participante incrementó su respuesta durante el inicio de las fases B sin disminuir durante la primera parte (viable) de la condición C, sugiriendo un nivel pre-conciente. Él mostró P300 y respuestas de discordancia negativas indistintas y puntuación de nivel vegetativo en el CRS-R.

Conclusión: Los datos de aprendizaje aparentan concordancia con las mediciones neurofisiológicas y son más positivas que las puntuaciones de CRS-R.
