Summary
A description of a 17-year-old male patient with active juvenile rheumatoid arthritis who developed symptoms and signs simulating brain tumor. In carotid angiography there could be seen caliber changes in the vessels of the arteria media group and a distinct expansive process in the left hemisphere. The patient received prednisone, and in a control angiography taken six weeks later the angiographic finding was normal. The rheumatoid inflammatory process remained active during the observation period of one and a half years, but the neurological signs diminished. Rheumatoid factor tests and tests for antinuclear factors were negative, but the patient had a marked increase of immunoglobulins.

Les auteurs décrivent le cas d'un malade masculin âgé de 17 ans atteint de polyarthrite rhumatoïde juvénile dont les symptômes simulaient la présence d'un encéphalome. A l'aide de l'artériographie des carotides il a été possible de constater des changements de calibre des vaisseaux du groupe de l'artère médiane et un processus expansif significatif dans l'hémisphère gauche. Le patient fut traité à la prédnisone et dans une artériographie entreprise après six semaines à titre de contrôle le résultat de l'examen du malade fut normal. Le processus inflammatoire rhumatismal resta actif durant la période d'observation d'un an et demi, toutefois les symptômes neurologiques de la maladie subirent un affaiblissement. Les épreuves effectuées pour déterminer le facteur rhumatoïde et les facteurs anti-nucléaires ont été négatives, tandis que le malade subit un accroissement marqué de l'immunoglobuline.

Beschreibung eines 17-jährigen, männlichen Patienten mit juveniler chronischer Gelenkentzündung, bei der Symptome und Anzeichen auftraten, die Gehirntumor vortäuschten. Bei Arteriographie carotis konnten Veränderungen der Lichtweite in den Gefässen der Arteria media-Gruppe und ein eindeutig expansiver Prozess in der linken Hemisphäre festgestellt werden. Der Patient erhielt Prednison, und in einer nach sechs Wochen zu Kontrollzwecken vorgenommenen Arteriographie war der arteriographische Befund normal. Der rheumatische Entzündungsprozess verblieb während der Beobachtungszeit von eineinhalb Jahren aktiv, doch schwächten sich die neurologischen Krankheitssymptome ab. Tests zur Feststellung des rheumatoiden Faktors und antinukleärer Faktoren verliefen negativ, doch zeigte sich bei dem Patienten ein deutliches Ansteigen des Immunoglobulins.
