Osteoporosis in Rheumatoid Arthritis: A Follow-up Study

Summary

During a follow-up period of 4 years 33 patients suffering from rheumatoid arthritis were repeatedly examined radiologically and with clinical and laboratory tests. Owing to the slow progress of the disease in most cases, the results obtained at the starting point and at the end point of the observation time were used in the statistical analysis. There was a highly significant correlation between the progress of osteoporosis of the proximal phalanges and the grip power of the hands as well as between the development of osteoporosis and destructive changes. Parallelity was also noted between the development of osteoporosis and sedimentation rate. Corticosteroid treatment lasting more than one year did not increase the pace of demineralisation of the hand bones.

Résumé

Durant une période de près de 4 années 33 patients souffrant de polyarthrite chronique évolutive ont été examinés à plusieurs reprises radiologiquement et avec-des épreuves cliniques et de laboratoire. étant donné la lente progression de la maladie dans la plupart des cas, les résultats obtenus au début et à la fin de la période d'observation ont été utilisés dans l'analyse statistique. Les auteurs constatèrent une corrélation hautement significative entre la progression de l'ostéoporose des phalanges proximales et le pouvoir d'étreinte des mains ainsi qu'entre le développement de l'ostéoporose et les altérations destructives. La parallélité fut également notée entre le développement de l'ostéoporose et la vitesse de sédimentation. Un traitement avec corticostéroïdes durant plus d'un an n'a pas augmenté l'allure de déminéralisation des os de la main.

Zusammenfassung

Während eines fortlaufenden Zeitraumes von 4 Jahren wurden 33 an chronischer Gelenkentzündung leidenden Patienten wiederholt röntgenologisch und mittels klinischer und Laborversuchen untersucht. Infolge des langsamen Fortschreitens der Krankheit in den meisten Fällen wurden die Ergebnisse zu Beginn und am Ende der Beobachtungsfrist in der statistischen Analyse benutzt. Es bestand eine sehr eingehende Beziehung zwischen dem Fortschreiten der Osteoporose der proximalen Phalanxen und der Fingerkraft sowie zwischen der Entwicklung der Osteoporose und den destruktiven Veränderungen. Eine übereinstimmung wurde auch zwischen der Entwicklung der Osteoporose und der Blutsenkungsgeschwindigkeit festgestellt. Eine über ein Jahr andauernde Kortikosteroidbehandlung erhöhte nicht das Demineralisierungstempo der Handknochen.
