In the British medical system, the M.D. is considered an advanced degree that is conferred after a physician has begun practicing medicine and has submitted a dissertation.

For the exchange of letters in which Bell is mentioned by Stevenson and Doyle, see the former's 5 April 1893 letter to Doyle in The Letters of Robert Louis Stevenson, H: 49-50; 8: 50n. Doyle also acknowledged his debt to Bell, in more polite language, in his ,Memories and Adventures (89), and he dedicated the 1892 Adventures of Sherlock Holmes to him. Bell, who had made Doyle his outpatient clerk, was clearly pleased to be identified as a "source" for his former student's famous detective, and the surgeon wrote an article on "Mr. Sherlock Holmes" for the London Bookman (later reprinted as an introduction to A Study in Scarlet) in which he praised Doyle and said his medical training had been an excellent preparation for his creation of Holmes's modus operand!, since the "precise and intelligent recognition … of minor differences are the real essential factors in all successful medical diagnosis."

But the nickname becomes "a title of honor" in the small village where he practices medicine after he proves himself to be a "capable surgeon and an accomplished physician." Significantly, this doctor, like Verrinder Smith, is a graduate of a Scottish University-in his case, Glasgow, , n.d.)
