Evaluation of NRT and behavioral measures for MAPping elderly cochlear implant users

Abstract

We investigated the acceptability of electrophysiologically derived MAPs and the effect of these MAPs on speech perception in elderly adults using Nucleus 24 cochlear implants. Eight implant recipients aged 75 years or older trialed an electrophysiologically derived MAP and a behavioral MAP. The electrophysiologically derived MAP was based on the threshold and maximum comfort level for electrode 10 and evoked compound action potential thresholds measured on six electrodes using neural response telemetry (NRT). Word perception at 55 dB SPL and sentence perception in noise at 70 dB SPL were assessed after six weeks take-home experience and again after an additional two weeks of experience. During the final two weeks of take-home experience participants indicated their preferred MAP for different listening situations. The NRT derived MAP estimated behavioral T levels well, but underestimated behavioral C levels for apical electrodes in some subjects. Speech perception with NRT derived MAPs was comparable to speech perception with behaviorally measured MAPs. MAPs estimated from NRT data provided good speech perception outcomes for elderly implant recipients and were well tolerated.

Investigamos la aceptación de los mapas obtenidos electrofisiológicamente y el efecto de los mismos en la percepción del lenguaje en adultos mayores que usan implantes cocleares Nucleus 24. En ocho de ellos, de 75 o más años de edad, se hicieron mapas por medios electrofisiológicos y conductuales. Los primeros se basaron en el umbral y en el nivel de máxima comodidad en el electrodo 10 así como en el potencial evocado de acción compuesto medido en seis electrodos, por medio de la telemetría de respuesta neural (NRT). Se valoró la percepción de palabras a 55 dB SPL y la percepción de oraciones en ruido a 70 dB SPL, después de seis semanas de experiencia en el hogar y posteriormente, dos semanas después. Durante las dos últimas semanas de experiencia en el hogar, los participantes indicaron que preferían el mapa para diferentes situaciones de escucha. El mapa derivado de la NRT estimó bien los niveles T conductuales pero subestimó los niveles C conductuales en los electrodos apicales en algunos sujetos. La percepción del lenguaje con los mapas derivados de la NRT fue comparable a la percepción del lenguaje obtenida con medidas conductuales. Los mapas estimados con los datos de la NRT dieron buenos resultados para la percepción del lenguaje en usuarios de la tercera edad y fueron bien tolerados.
