An abnormal concentration of cases of Rendu-Osler disease in the Valserine valley of the French Jura: a genealogical and demographic study

Summary

An important concentration of patients with Rendu-Osler disease occurs in the Valserine valley of the French Jura. A study of marriages shows that, in spite of its somewhat remote location, the valley cannot be considered an isolate, but that, on the contrary, exogamy is widely practised. Only 17·8% of the genes of inhabitants during the present century can be traced to the original population, but persons affected with the disease belong to a subgroup of the population which has lived in the villages for more than 10 generations. All the patients in 85 sibships are related. The smallest number of originator couples who lived at the beginning of the eighteenth century amounts to 16, the unique originator may therefore have lived approximately four generations earlier. However, as other patients who lived at the periphery of the Valserine valley do not appear to be related to any patients living in the valley, and because there has been considerable immigration into the valley, a number of hypotheses to explain the distribution of the disease in the region remain possible.

Zusammenfassung

Im Valserinetal des französischen Jura tritt eine deutliche Konzentration von Patienten mit Rendu-Osler-Syndrom auf. Eine Analyse der Eheschließungen zeigt, daß im etwas entlegenen Lage das Tal nicht als Isolat betrachtet werden kann, sondern daß im Gegenteil Exogamie weithin praktiziert wird. Nur 17·8% de Gene der Bewohner des Tales in diesem Jahrhundert können auf die Ursprungspopulation zurückgeführt werden; Personen, die von der Krankheit betroffen sind, gehören jedoch zu einer Untergruppe der Population, die seit mehr als 10 Generationen in den Dörfern gelebt hat.

Alle Patienten der erfaßten 85 Familien sind verwandt. Die kleinste Zahl von Ursprungspaaren, die zu Beginn des 18. Jahrhunderts gelebt haben, beträgt 16, der ursprüngliche Genträger könnte also etwa vier Generationen früher gelebt haben.

Da andere Patienten, die an der peripherie des Valserinetals leben, jedoch nicht mit patienten aus dem Tal verwandt zu sein scheinen, und da es eine beträchtliche Immigration in dieses Tal gegeben hat, sind immer noch eine ganze Reihe von anderen Möglichkeiten zur Erklärung der Verteilung dieser Krankheit denkbar.

Une importante concentration de patients atteints de la maladie de Rendu-Osler, est présente dans la vallée de la Valserine dans le Jura français. L'étude des mariages montre qu'en dépit de son relatif isolement géographique, la vallée ne peut ˆetre considérée comme un isolat, mais qu'au contraire, l'exogamie y est largement pratiquée. Seulement 17,8 pour cent des g`enes des habitants du si`ecle présent, peuvent ˆetre rattachés `a la population originale et les personnes affectées par la maladie appartiennent `a un sous-groupe de la population ayant vécu dans les villages pendant plus de dix générations. Tous les patients de 85 fratries sont apparentés. Le plus petit nombre de couples de leur ascendance qui peut ˆetre retracé est de 16 au début du 18`eme si`ecle, l'introducteur unique de la maladie peut d`es lors avoir vécu approximativement quatre générations plus tˆot. Cependant, comme d'autres patients qui vécurent `a la périphérie de la vallée de la Valserine ne sont apparentés `a aucun des patients vivant dans la vallée et parce qu'il y a eu une immigration considérable dans la vallée, de nombreuses hypoth`eses peuvent expliquer la distribution de la maladie dans la région.
