Fitting strategies and candidature criteria for unilateral and bilateral hearing aid fittings

Abstract

In this paper, hearing aid fitting strategies are reviewed and candidature criteria for unilateral and bilateral fittings are discussed. Fitting strategies are primarily governed by the objectives of hearing aid fitting that can be set in different ways. Based on common goals, fitting strategies can be broken down into three stages: (1) basic fitting, (2) fine-tuning and (3) verification and validation. A fine-tuning concept basing on so-called meta-controllers is described and the issue of bilateral fitting strategies is discussed. In a first order approach, bilateral hearing aids can be fitted in parallel as unilateral aids, followed by bilateral fine-tuning, making sure that loudness is balanced and overall loudness is accepted. Parallel fitting of bilateral aids is more favourable than sequential fitting to avoid rejection because of a negative experience with unilateral amplification due to difficulties in disadvantageous listening situations. Candidature criteria are presented by means of a flow chart structuring the decision making process and arriving at the conclusion that bilateral fitting should be considered the normal case. If there is any doubt about the benefit of bilateral fitting, a trial with bilateral aids should be performed.

En este trabajo se revisan las estrategias de adaptación de auxiliares auditivos y se discuten los criterios de candidatura para adaptación uni o bilateral. Las estrategias de adaptación están regidas principalmente por los objetivos de la adaptación de auxiliares auditivos que pueden establecerse de diferentes formas. Basados en una meta común, las estrategias de adaptación pueden dividirse en tres etapas: (1) adaptación básica, (2) afinación precisa y (3) verificación y validación. Se describe un método de afinación precisa basado en los llamados meta-controladores y se discute el tema de las estrategias de adaptación bilateral. En un acercamiento inicial, se pueden adaptar los auxiliares auditivos en paralelo como auxiliares unilaterales y a continuación se realiza el afinamiento bilateral preciso, asegurándose que la sensación de intensidad está balanceada y sobre todo, que es aceptable. La adaptación bilateral paralela de auxiliares es más favorable que la adaptación secuencial para evitar el rechazo derivado de una experiencia negativa con amplificación unilateral, debida a su vez a dificultades en situaciones sonoras desventajosas. Se presentan los criterios de candidatura en un diagrama de flujo que estructura el proceso de toma de decisiones y que llega a la conclusión de que la adaptación bilateral de auxiliares debe ser considerada como el caso normal. Si existe alguna duda sobre el beneficio de la adaptación bilateral de auxiliares, se debe realizar una prueba clínica con dos auxiliares.
