When this article first published, the number of maternal deaths was based on preliminary findings from the CDC RAMOS study. The final data have now been supplied and the article has been updated accordingly.

The number of maternal deaths given in the Abstract and third paragraph of the Introduction has been changed from 77 to 52. The number of pregnancy-related deaths has also been added to the third paragraph of the Introduction.
